Logistics Shipment Pick Up and Delivery Form

The Shipment Pick Up and Delivery Form is linked to an email notification for the contact at the shipper.   The form confirms the pickup of the shipment and asks general satisfaction questions about the driver and other related service questions.  If a negative response is received, points are added to the carrier's scorecard and a workflow is generated for a CSR to call the shipper.  Flags can be updated in the dispatch system to prevent the carrier from being dispatched to that shipper or the information can be stored in the SHIPS system for future review.  If a shipper does not respond to the Pick-Up Form by a given timeframe, a workflow is generated to call the shipper to confirm the information.  The same process is configured to send a Delivery Form.  If an OS&D claim is indicated on the form, a CSR is notified and a settlement hold is placed on the order number.  A notification of the OS&D is sent to the carrier with the instructions on who to conact to begin the resolution process.
